Job summary

CVS Health, in partnership with Signify Health, is seeking a Per Diem Physician to conduct in-home health evaluations for members in their own homes, delivering personalized health assessments and bridging gaps in care. The role emphasizes compassionate patient engagement and adherence to health plan guidelines.

The position is part-time, with a typical 12 hours per week, and requires current, unrestricted licenses.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses preferred.

Responsibilities

• Conduct in-home health evaluations (~45 minutes)
• Perform medication and medical history reviews
• Perform physical evaluations and select diagnostic tests when ordered by health plan
• Travel locally and within licensed states as needed
• Conduct virtual visits when needed
• Provide Signify Health services including diagnostic and preventive services
