Job summary

Giving Home Health Care in Alcalde, NM seeks a compassionate Nurse (RN or LPN) for in-home care under a physician care plan. This 1099 contract role involves 4 hours weekly, with 2 visits of 2 hours or a single 4-hour visit on Fridays, at the patient's home.

You will work one-on-one with patients, coordinate with CNAs, case managers, and physicians, and provide education to families. Background checks are required.

Qualifications

• Graduate of an accredited college or university with RN or LPN licensure.
• Current state or compact RN/LPN license.
• Current CPR certification.
• TB test and physical examination.
• Maintain required insurance.
• Ability to stand or walk for prolonged periods and lift up to 50 lbs.
• Willingness to commute to patients' homes.

Responsibilities

• Execute physician prescribed plans of care.
• Document care in the system of record.
• Administer prescribed medications and therapies.
• Conduct patient assessments and coordinate care.
• Assist with activities of daily living.
• Monitor vitals and intake/output.
• Assess skin integrity and provide wound care.
• Communicate with family, physicians, and case managers.
• Educate patients and families.
• Provide one-on-one care in the home.
• Supervise CNAs and PCAs.
• Perform other personal care services as needed.
