Job summary

Qualifications

Responsibilities

• Execute physician prescribed plans of care.
• Document care provided accurately in the system of record.
• Administer medications and therapies as prescribed.
• Assess patient condition and coordinate care with team.
• Provide activities of daily living and skin/wound care as needed.
• Monitor vitals and report changes to supervisors.
• Educate patient and family; collaborate with CNAs/HHA.
