Qualifications

• Graduate of an accredited college or university with an RN or LPN credential.
• Current State or Compact Nursing license.
• TB test and physical examination.
• CPR certification is active.
• Maintain required insurance.
• Good interpersonal skills.
• Ability to stand/walk for long periods and lift up to 50 lbs.
• Willingness to commute to patient homes.
• Ability to provide basic medical management in home settings.

Responsibilities

• Execute physician prescribed plans of care.
• Document care accurately in the system of record.
• Manage and administer prescribed medications and therapies.
• Conduct patient assessments and coordinate care.
• Support patients with activities of daily living.
• Monitor vitals and intake/output; watch for changes in condition.
• Provide wound care and monitor skin health.
• Communicate with family, physicians, and case managers.
• Educate patients and caregivers; train home health aides.
