Job summary

Giving Home Health Care in Alcalde, NM is seeking a compassionate Registered Nurse or Licensed Practical Nurse for in-home patient care under a physician care plan. This 1099 contract role focuses on building lasting caregiver-patient relationships, delivering personalized care at the patient's residence.

Schedule: 4 hours per week (Mondays or Fridays) with flexible two-visit options or a single 4-hour Friday slot. Rate ranges: LPN up to $35/hr; RN up to $50/hr, depending on experience.

Qualifications

• Graduate of an accredited college or university with an Associate or Baccalaureate Nursing degree.
• Current State or Compact RN/LPN license.
• Current CPR certification.
• TB test and physical examination.
• Maintain required insurance.
• Good interpersonal skills.
• Ability to lift up to 50 lbs and move patients.

Responsibilities

• Execute physician prescribed plans of care.
• Complete compliant documentation of care provided.
• Manage and administer prescribed medication and therapies.
• Conduct patient assessments and coordinate care.
• Assist with activities of daily living.
• Monitor vitals and track GI intake/output.
• Assess skin integrity and administer wound care.
• Communicate with family, physicians, case managers about patient health.
• Provide education and training to patient/family/Home Health Aide.
• Collaborate with and supervise CNAs and Personal Care Attendants.
