Job summary

Giving Home Health Care in Espanola, NM seeks a compassionate RN or LPN for in-home patient care on a 1099 contract. Expect 18 hours per month, with visits on Tuesdays and Wednesdays starting at 6pm. You will follow physician plans, document care, and collaborate with CNAs, HHAs, case managers, and physicians.

We offer a supportive team and emphasize flexible, patient-centric care. Applicants must hold an active nursing license, CPR, and be able to lift 50 lbs while commuting to patient homes.

Qualifications

• Graduate of an accredited college or university with anAssociate or Baccalaureate School of Nursing.
• Current State or Compact RN/LPN license.
• Current CPR certification.
• TB test and physical examination.
• Maintain required insurance.
• Ability to lift up to 50 lbs and move patients.
• Must have dependable transportation and commute to patient homes.
• Ability to provide basic medical management up to end-of-life care.

Responsibilities

• Execute physician prescribed plans of care.
• Complete compliant documentation in the system of record.
• Manage prescribed medications, treatments, and therapies.
• Conduct patient assessments and coordination of care.
• Provide activities of daily living and monitor vitals.
• Communicate with family, physicians, and case managers.
