Job summary

Giving Home Health Care is seeking a compassionate RN or LPN for in-home, 1099 nursing care in Alcalde, NM. The role involves delivering patient-centered care per physician plans, collaborating with CNAs and case managers, and documenting thoroughly in our system.

Expect 9-hour shifts (6am–3pm), Monday–Saturday, with competitive hourly rates. RN up to $55 and LPN up to $35, depending on experience; background check required.

Qualifications

• Graduate of an accredited college or university; Associate or Baccalaureate School of Nursing.
• Current State or Compact License as RN or LPN.
• Current CPR certification.
• TB test and physical examination.
• Maintain required insurance.
• Good interpersonal skills.
• Hand-eye coordination and manual dexterity.
• Ability to stand or walk for prolonged periods; lift up to 50 lbs and move patients.
• Dependable transportation; willing to commute to patients’ homes.
• Ability to provide basic medical management up to end-of-life care.

Responsibilities

• Execute physician-prescribed plans of care.
• Complete compliant documentation in the system of record.
• Manage and administer prescribed medication, treatment, and therapies.
• Conduct patient assessments and coordinate care.
• Monitor vitals and GI intake/output.
• Assess skin integrity and administer wound care.
• Monitor changes in patient condition and progression indicators.
• Communicate with family, physicians, case managers, and other care providers.
• Provide education and training to patient, family, and Home Health Aide.
• Work one-on-one in patients' homes to ensure comfort.
• Supervise CNAs and Personal Care Attendants.
• Perform other personal care services to meet patient needs.
