Job summary

Signify Health, part of CVS Health, seeks a Part-Time Clinician (NP or PA) to provide in-home health evaluations. Each visit lasts about 45 minutes, including health questions, medication/history review, and a brief exam.

You will travel locally and within licensed states, conduct virtual visits when needed, and join a national clinician network dedicated to bridging care gaps. Active NP/PA licensure with board certification is required.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• In-home evaluations lasting about 45 minutes with health questions, medication/history review, and a physical exam.
• Travel locally and within licensed states; conduct virtual visits as needed.
• Join a national clinician network dedicated to improving health outcomes.
• Provide diagnostic & preventive services as indicated within program guidelines.
• Foster connections to the right care for members’ unique needs.
• Adhere to safety, quality, and regulatory requirements throughout visits.
