Signify Health, part of CVS Health, seeks a Part Time Clinician (NP or PA) to perform in‑home health evaluations across communities. Each visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and any necessary on‑site tests ordered by the health plan.

Clinicians use an iPad and a streamlined workflow and are not required to prescribe or alter existing regimens.

Qualificações

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsabilidades

• Bring your heart into every visit, joining a national network of clinicians dedicated to improving health outcomes.
• Visit millions of people nationwide with the goal of connecting to the right care for each member.
• Work with Signify Health as a clinician colleague.
• Be flexible to travel locally and within licensed states.
• Conduct virtual visits as needed.
• Provide Diagnostic & Preventive Services (e.g., spirometry, DEE, bone density) as indicated.
