Job summary

Signify Health, part of CVS Health, is seeking a Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations. Each visit lasts about 45 minutes and includes medication and medical history review, a physical exam, and, if ordered, additional diagnostic tests.

You will travel locally to homes within licensed states, conduct virtual visits when needed, and join a national network of clinicians dedicated to improving health outcomes.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Provide in-home health evaluations as part of Signify Health services.
• Travel locally within licensed states and conduct virtual visits as needed.
• Collaborate with Signify Health clinicians to deliver Diagnostic & Preventive Services.
