Job summary

Signify Health, part of CVS Health, seeks a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations. Visits run about 45 minutes, with the clinician using an iPad and a streamlined workflow.

Travel locally and within licensed states is required, and virtual visits are possible. Active, unrestricted NP/PA licenses are required, board certification is needed, and multi-state licenses are preferred.

Responsibilities

• Conduct in-home health evaluations (≈45 minutes)
• Engage with patients and answer health questions
• Use iPad and clinical workflow during evaluations
• Travel locally and within licensed states as needed
• Conduct virtual visits when required
• Provide Signify Health services as indicated (diagnostic & preventive)
