Signify Health, part of CVS Health, is seeking a Part Time Clinician (NP or PA) to provide in-home health evaluations. Visits are conducted in members’ homes, with a 45-minute visit and a focus on bridging gaps in care.

Qualifications

• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Conduct in-home health evaluations for health plan members.
• Review medication history and perform physical evaluation.
• Travel locally to licensed states as needed and conduct virtual visits.
