Job summary

Matrix Medical Network is seeking a qualified NP to perform comprehensive health assessments in patients’ homes or SNFs, contributing to improved care quality through CHA visits. Expect collaboration with clinicians, case managers, and family members to ensure thorough health evaluations and documentation.

Qualifications

• Master’s Degree or commensurate experience and NP licensure
• Current NP licensure in state of practice with prescriptive authority
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
• Current BLS, ACLS or CPR certification
• 1 year of NP experience preferred, new grads encouraged to apply
• Valid state driver’s license for the state of residence
• Automobile insurance coverage for the state of residence (or ability to obtain)
• Strong computer skills and familiarity with employee health/medical record software
• Excellent verbal and written communication skills

Responsibilities

• Conduct adult / geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation per Matrix guidelines
• Administer and order point-of-care and lab tests as needed
• Collaborate with physicians, case managers, social workers, family/caregivers, and other personnel as appropriate
• Coordinate with PCP on patient education and follow-up
• Provide services at the member’s residence (homes or Skilled Nursing Facilities)
