Zusammenfassung

Matrix Medical Network seeks licensed nurse practitioners to perform comprehensive health assessments at members’ homes nationwide. Each visit may last up to one hour and includes review of medical history, medications, and risk factors to close care gaps.

Providers administer point-of-care and lab tests, coordinate with physicians and care teams, and document findings per Matrix guidelines. A valid NP license, board certification, and state-insured driving are required; sign-on bonus available.

Qualifikationen

• Active NP license in practicing state with prescriptive authority
• Board certification in Adult, AGPCNP, AGACNP, Family, ER, or Acute Care
• BLS/ACLS/CPR certification required
• 1 year NP experience preferred; new grads encouraged to apply
• Valid driver’s license and auto insurance per state requirements

Aufgaben

• Conduct adult/geriatric assessments with medical history, diagnosis and treatment per Matrix guidelines
• Administer point-of-care and lab testing as needed
• Collaborate with physicians, case managers, social workers, families and caregivers
• Coordinate with PCP on patient education and follow-up
• Provide services at the member’s residence (home or SNF)
