Signify Health, Inc. is hiring physician contractors (MD/DO) to conduct in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members.

You will perform in-home visits to assess overall health and identify barriers to care, reviewing medications and health history in the member’s home. Compensation ranges from $3,000 to $5,000 per week (average 6–9 visits daily at a base rate of $100 per completed assessment), with potential increases up to 135% with the Market

Qualifications

• Physician contractors (MD/DO) for in-home health evaluations.
• Medicaid license or willingness to enroll preferred with support.

Responsabilités

• Conduct in-home visits focusing on overall health and well-being.
• Review medication history and perform a physical evaluation in the home.
• You will not prescribe medications or order labs or alter treatment regimens.
• Use pre-populated patient-specific forms for each evaluation.
