Signify Health, Inc. is seeking physician (MD/DO) contractors to conduct in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plans.

Contractors conduct in-home visits, review medications and medical history, perform physical evaluations, and document findings. Expected earnings are up to $3,000–$5,000 per week with base $100 per completed assessment and potential multiplier increases.

Qualifications

• MD/DO with valid medical license.
• Medicaid license preferred or willing to enroll with our support.

Responsabilités

• Conduct in-home visits focusing on overall health and well-being and identify barriers to care.
• Perform in-home medication and medical history review and a physical evaluation.
• Document findings for health plans and coordinate with care teams.
