Job summary

Signify Health, Inc. is recruiting physician contractors to perform in-home health evaluations for Medicare Advantage, Medicaid, and selected commercial plan members.

You will conduct visits in patients’ homes, review medical histories, and provide careful assessments as part of the health plan care team. This role offers flexible scheduling, with additional compensation through our pay-per-test model and potential market multipliers.

Qualifications

• Licensed physician capable of practicing medicine in applicable state.
• Willingness to enroll in Medicaid with company support.
• Ability to conduct in-home visits and comprehensive evaluations.

Responsibilities

• Conduct in-home visits focusing on overall health and barriers to care.
• Review medication history and perform physical evaluation in patients' homes.
• Generated reports and discussions with health plan members and care teams.
