Job summary

Benefis Health System is seeking a Home and Community-Based Case Manager to coordinate patient-centered services in home and community settings. The role focuses on clinical assessment, medication review, and management of chronic illnesses to prevent hospitalizations and ensure medical stability at home.

You will be part of a multidisciplinary Care Management Team serving Montana Big Sky Waiver Medicaid beneficiaries, requiring strong collaboration, crisis intervention skills, and knowledge of

Qualifications

• Graduate of an accredited school of nursing; RN or LPN as state approved
• Current state licensure as RN or LPN
• Certification in Case Management preferred
• Experience as a nurse (RN/LPN) with at least 3 years of professional nursing
• Knowledge of case management methods, practice and procedures
• Knowledge of Medicare/Medicaid reimbursement processes
• Knowledge of issues and needs of long-term care consumers
• Prior knowledge of managing a budget

Responsibilities

• Deliver home and community-based case management services as part of the Care Management Team
• Coordinate assessment, planning, and monitoring of services for Medicaid beneficiaries
• Review medications, manage chronic illnesses, and coordinate skilled services
• Adhere to licensing board requirements and Benefis Health System policies
• Handle deadlines and adapt to changing situations while maintaining accuracy
