Qualifications

• Graduate from an accredited nursing school; BSN preferred or LPN acceptable.
• Must have current state licensure as a Registered Nurse or LPN.
• Minimum of three years of professional nursing experience.

Responsibilities

• Provide clinical assessment, medication review, and chronic illness management.
• Coordinate services to prevent hospitalization and support stability at home.
• Develop individualized care plans as part of the Care Management Team.
