Benefis Health System in Bozeman, Montana is seeking a Home and Community-Based Case Manager to provide clinical assessments and coordinate care for Medicaid beneficiaries. This role requires a graduate from an accredited nursing school and three years of nursing experience.

The ideal candidate will have strong skills in chronic illness management and case management. A remote option is available for those living in or around Gallatin County, offering a Monday through Friday schedule without weekend or call duties.

Qualifications

• Graduate from an accredited nursing school; BSN preferred or LPN acceptable.
• Must have current state licensure as a Registered Nurse or LPN.
• Minimum of three years of professional nursing experience.

Responsabilités

• Provide clinical assessment, medication review, and chronic illness management.
• Coordinate services to prevent hospitalization and support stability at home.
• Develop individualized care plans as part of the Care Management Team.
