HarmonyCares is seeking aValue-based Advanced Practice Provider (NP/PA) to manage high-risk geriatric patients in home settings across Monday–Friday. A $10,000 signing package is offered with a salary range of $135k–$155k per year, plus benefits and a quality-based bonus.

Qualifications

• Licensed as NP or PA in the state of practice.
• Federal DEA license/eligibility.
• CPR/BLS certification required.
• Board certification in family medicine or geriatrics preferred.
• Must maintain a valid driver’s license and clean driving record.

Responsibilities

• Provide in-home primary care to high-acuity geriatric patients.
• Focus on patient-centered care over volume and time efficiency.
• Collaborate with interdisciplinary team to coordinate home health services.
