Qualifications

• Licensed NP in the state where HarmonyCares Medical Group office is located.
• Federal DEA License/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Provide home-based primary care to high acuity geriatric patients.
• Address patient needs with time and thorough assessments.
• Collaborate with care teams to deliver patient-centered care.
