Job summary

HarmonyCares seeks a Nurse Practitioner or Physician Assistant to deliver home-based primary care for high-acuity Medicare patients. You will work with an interdisciplinary team to address patients’ needs with time and compassion, prioritizing quality of care over volume.

With competitive base salary plus annual quality-based bonus, the role offers a 401K match, CME stipend, licensing dues, and malpractice tail coverage. Travel to patient homes is required within Texas markets.

Qualifications

• Licensed as a NP or PA in the State where HarmonyCares Medical Group office located.
• Federal DEA License/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Directly related experience in home health, geriatrics, hospice and primary care.
• Provide patient-centered care with time to address needs.
• Meet patients where they are via home visits.
