HarmonyCares is a national value-based home-based primary care provider focused on high-acuity geriatric patients. This Advance Practice Provider role is M-F, with hours roughly 8:00 a.m. to 4:30/5:00 p.m., no weekends or evenings.

Charting is completed during the workday using Athena EMR plus Marvix AI Scribe and a physician-driven care model. The position emphasizes patient-centered care over volume, offering a competitive base salary and comprehensive benefits, with minimal call

Qualifications

• Licensed as a NP/PA in the state where HarmonyCares Medical Group office located.
• Geriatric training/experience preferred.
• Board Certification required.
• Active CPR/BLS Certification.
• Federal DEA License/eligibility.
• Valid and unencumbered driver's license with good driving record.

Responsabilités

• Provide in-home primary care to high-acuity geriatric patients.
• Manage care with a patient-centered approach, focusing on addressing patient needs.
• Practice within a value-based, physician-driven care model and minimize hospital rounds.

Formation

NP/PA licensure in the applicable state
