Job summary

HarmonyCares is seeking a Nurse Practitioner or Physician Assistant to deliver in-home primary care for a high-acuity geriatric patient panel in a value-based team setting. This Monday–Friday role avoids weekends and evenings, with dedicated support to manage chronic conditions.

You will collaborate with an interdisciplinary team and focus on patient-centered, quality care in the home. Qualifications include state NP/PA licensure, DEA, CPR/BLS, board certification, and a valid driver's license;

Qualifications

• Licensed NP or PA in the state of practice.
• DEA license/eligibility required.
• Geriatric care experience preferred.
• CPR/BLS certification active.
• Board certification required.
• Valid driver’s license with good driving record.

Responsibilities

• Provide in-home primary care to high-acuity geriatric patients.
• Focus on assessment, care planning, and chronic condition management.
• Collaborate with interdisciplinary team in value-based care model.
