FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one care to older adults in Cream Ridge, NJ, in private residences or Senior Living Communities.

You will conduct evaluations, create plans, and provide evidence-based therapy while building lasting patient relationships. Flexibility, autonomy, and the option to incorporate telehealth enhance patient access and outcomes.

Competenze

• Valid Physical Therapy license in state(s) of practice or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Mansioni

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, memory care)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support wellness, mobility and independence
• Document effectively in FOX’s EMR system per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth or virtual care to extend access and flexibility
