FOX Rehabilitation seeks licensed Physical Therapists to deliver one-on-one geriatric care in patients’ homes across Succasunna, NJ and senior living communities. You’ll develop personalized treatment plans, perform evaluations, and help patients improve mobility and independence through the Geriatric House Calls™ model.

Flexibility to create your own schedule and integrate telehealth as needed, with opportunities to work across private residences, memory care, and senior living settings.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsabilités

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, etc.)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility
