FOX Rehabilitation in Williamstown, NJ seeks a licensed Physical Therapist to deliver one-on-one geriatric care in homes and senior living communities. Treat patients, develop personalized plans, and document in FOX's EMR while collaborating with an interdisciplinary team.

This role offers autonomy, a flexible schedule, and opportunities to use telehealth to extend care. New graduates welcome; in-home care model enables meaningful patient relationships.

Qualifications

• Valid Physical Therapy license in NJ or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsabilités

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support mobility, independence, and safety.
• Document in FOX's EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff on safety and aging-in-place.
• Provide telehealth/virtual care when appropriate.
