Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one care to older adults in their homes across New Jersey, including private residences and senior living communities, under the Geriatric House Calls model.

The role offers autonomy, flexible scheduling, and the chance to build lasting patient relationships, with the option to incorporate telehealth as part of care.

Qualifications

• Licensed Physical Therapist in NJ or eligibility to obtain license.
• Degree from an accredited Physical Therapy program (DPT or equivalent).
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence and function.
• Document within FOX's EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs and Fitness Specialists.
