Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to provide one‑on‑one geriatric care in homes and senior living communities in South Plainfield, NJ. You will deliver personalized treatment plans and use our Geriatric House Calls™ model to improve mobility and independence.

Clinicians may incorporate telehealth when appropriate to extend access and flexibility, while maintaining autonomy over scheduling and dedicated patient relationships.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one‑on‑one physical therapy to older adults wherever they call home (private residences, senior living communities).
• Perform evaluations, develop individualized plans of care, and deliver evidence‑based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety and aging in place.
