FOX Rehabilitation is seeking licensed Physical Therapists to provide one‑on‑one geriatric care in Allentown, NJ, including private residences and senior living communities. Youll evaluate, plan, and deliver evidence‑based therapy, collaborating with an interdisciplinary team and documenting in FOXs EMR system.

The role emphasizes autonomy, flexible scheduling, and potential telehealth integration to extend access and enhance continuity of care for older adults and their caregivers.

Qualifikationen

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Aufgaben

• Provide one-on-one physical therapy to older adults wherever they call home (private residences or senior living communities).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access and flexibility.
