Job summary
FOX Rehabilitation is expanding in Allentown, PA and seeks licensed Physical Therapists to deliver one-on-one care in homes and senior living communities. You’ll evaluate, plan, and deliver evidence-based therapy while documenting in FOX’s EMR.

Qualifications
- Active Physical Therapy license in PA or eligibility to obtain.
- Experience with geriatric populations and home-based care.
- Proficient in EMR documentation and electronic records.
- Current CPR certification and ability to meet physical requirements.
Responsibilities
- Provide one-on-one physical therapy to older adults in homes and senior living settings.
- Perform evaluations, develop plans of care, and deliver evidence-based treatment.
- Deliver therapy sessions that improve mobility, independence and function.
- Document within FOX’s EMR system per regulatory standards.
- Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and staff to promote aging in place.
- Provide telehealth/virtual care when appropriate.
