Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one, in-home care to older adults in Allentown, PA. You will evaluate patients, create tailored treatment plans, and provide evidence-based therapy in residences and senior living settings.

Telehealth may be incorporated to enhance access and continuity of care. You’ll enjoy autonomy in scheduling, a collaborative team environment, and a mission-driven culture focused on meaningful outcomes and aging in place.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, etc.)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions to support wellness, mobility, and independence
• Document effectively within FOX's EMR system per regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access
