FOX Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one geriatric care in Tobyhanna, PA, including private residences and senior living communities. You’ll evaluate, create personalized plans, and provide evidence-based therapy, with the option to use telehealth when appropriate to extend access.

The role emphasizes autonomy, flexible scheduling, and meaningful patient relationships within FOX’s Geriatric House Calls model.

Qualifications

• Valid Physical Therapy license in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsabilités

• Provide one-on-one physical therapy to older adults in their homes or senior living communities.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to improve mobility and independence.
• Document in the EMR following regulatory standards.
