Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in Union City, NJ, across private residences and senior living communities. You’ll design plans of care, deliver evidence-based treatment, and document in FOX’s EMR while collaborating with PTs, OTs, SLPs, and Fitness Specialists.

The role offers autonomy, flexible scheduling, and opportunities to use telehealth as part of a geriatric house calls model.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in home, senior living communities, or similar settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve wellness, mobility, and independence.
• Document care effectively in FOX’s EMR system per regulatory standards.
• Collaborate with a interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Offer telehealth/virtual care to extend access when appropriate.
