Job summary

Fox Rehabilitation is seeking a licensed Physical Therapist to provide one-on-one care to older adults in private residences and senior living communities in Mount Arlington, NJ. You will evaluate and treat patients, develop plans of care, and document in FOX’s EMR system, with opportunities for telehealth integration.

The role emphasizes autonomy, flexible scheduling, and meaningful patient outcomes within a Geriatric House Calls™ model, with a supportive interdisciplinary team and growth

Qualifications

Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
Degree from an accredited Physical Therapy program.
Current CPR certification and basic computer/EMR proficiency.

Responsibilities

Provide one-on-one physical therapy to older adults in home or community settings.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Deliver therapy sessions to support mobility, independence, and wellness.
Document in the EMR system according to regulatory standards.
Collaborate with an interdisciplinary team and educate patients and caregivers.
