Job summary

Qualifications

• Active physical therapy license in state(s) of practice or eligibility.
• Degree from an accredited physical therapy program.
• Current CPR certification and basic computer/EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions supporting mobility, independence, and safety.
• Document in the EMR per regulatory standards and collaborate with an interdisciplinary team.
• Educate patients, caregivers, and staff to promote aging in place and functional outcomes.
