Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one care to older adults in Lexington, KY, in private residences or senior living communities. The role emphasizes autonomy, flexible scheduling, and meaningful patient outcomes through the Geriatric House Calls model.

Clinicians may incorporate telehealth and other virtual care formats, expanding access and continuity of care. The position offers full-time, part-time, and PRN pathways with benefits, and opportunities

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions to improve mobility, independence, and functional outcomes
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth / virtual care when appropriate to extend access
