FOX Rehabilitation is expanding its team of licensed Physical Therapists in Kansas City, MO, delivering one-on-one geriatrics care in patients' homes or senior living communities. You will evaluate patients, create individualized plans, and provide evidence-based treatments that improve mobility and independence.

Clinicians enjoy autonomy with flexible scheduling and opportunities to incorporate telehealth while collaborating with an interdisciplinary team for comprehensive care.

Qualifikationen

• Valid Physical Therapy license in the state(s) of practice or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Aufgaben

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support mobility, independence, and safety.
• Document effectively within FOX's EMR system.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients and caregivers to promote safety and aging in place.
