Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one care to older adults in St. Clair, MO and surrounding areas, either in private residences or Senior Living Communities.

You’ll improve mobility and independence through the Geriatric House Calls model while managing your own schedule. The role offers autonomy, flexibility, and meaningful patient outcomes, with options for in-home visits and telehealth approaches to extend access and continuity of care across

Qualifications

• Licensed Physical Therapist in the state or eligible for licensure.
• Current CPR certification and ability to work with older adults in home or community settings.
• Experience with in-home or geriatric care is preferred.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support mobility, independence, and safety.
• Document care in FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team to coordinate care.
• Educate patients and caregivers to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate to extend access.
