Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one care in clients' homes and senior living communities in Portland, ME. You’ll build strong patient relationships while managing your own treatment schedule in a Geriatric House Calls setting.

Ideal candidates value autonomy, flexibility, and meaningful outcomes, with opportunities to incorporate telehealth and virtual care to extend access and continuity of care.

Qualifications

• Licensed Physical Therapist in the state of practice (ME) or eligible to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Perform evaluations and develop individualized plans of care.
• Deliver evidence-based treatment to improve mobility and independence.
• Document effectively in the EMR per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate to extend access.
