Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one gait and movement therapies in Beaverton, OR. You’ll assess, plan, and execute care for older adults in homes or senior living settings, promoting independence and safety with an emphasis on geriatric outcomes.

This role offers autonomy, scheduling flexibility, and opportunities to integrate telehealth into regular care, with a collaborative, interdisciplinary team and ongoing professional growth.

Qualifications

• Active Physical Therapy license in Oregon or eligibility to obtain.
• Graduate from an accredited Physical Therapy program (DPT preferred).
• Ability to work autonomously in home-based settings.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatments.
• Document in FOX EMR per regulatory standards.
• Collaborate with an interdisciplinary team.
• Educate patients and caregivers to promote safety and aging in place.
• Provide telehealth when appropriate to extend care.
