FOX Rehabilitation is growing in Portland, ME, seeking licensed Physical Therapists to deliver one-on-one geriatric care in homes or Senior Living Communities. Clinicians balance autonomy with meaningful patient outcomes and flexible scheduling within the FOX Geriatric House Calls model.

Telehealth options are available to enhance continuity of care and reach more patients, while maintaining high-quality, individualized therapy that improves mobility and independence.

Qualifications

• Valid Physical Therapy license in state of practice or eligible to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsabilités

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver sessions to support mobility, independence, and function.
• Document in the EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate to extend access.
