Job summary

Fox Rehabilitation is expanding its Physical Therapy program in Brunswick, ME, delivering one-on-one care to older adults wherever they live—private homes or senior living communities—through the Geriatric House Calls model. Therapists set their schedules, maintain strong patient relationships, and may incorporate telehealth to extend access.

Ideal candidates are licensed Physical Therapists who value autonomy, flexibility, and meaningful patient outcomes while collaborating with PTs, OTs, SLPs,

Qualifications

• Valid Physical Therapy license in the state(s) of practice or eligibility to obtain.
• Current CPR certification.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.
