Job summary

FOX Rehabilitation, Concord, MA, is seeking licensed Physical Therapists to deliver one-on-one geriatric care in private residences and senior living communities through our Geriatric House Calls model.

You will evaluate mobility, create individualized plans of care, and deliver therapy to improve function while coordinating with an interdisciplinary team. Telehealth may be used to extend access and continuity of care.

Qualifications

• Valid Physical Therapy license in Massachusetts or eligibility to obtain.
• Graduate of an accredited Physical Therapy program (DPT preferred).
• Proficient in EMR systems and basic computer skills.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to improve mobility and independence.
• Document care in the EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate.
