Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to provide one-on-one geriatric care in Beaverton, OR, delivering in-home and senior living visits under our Geriatric House Calls model. You’ll evaluate, design plans, and treat with autonomy while building lasting patient relationships.

Flexibility to set your own schedule, optional telehealth, and collaboration with an interdisciplinary team make this role impactful, rewarding, and community-focused for older adults.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility

Education

DPT/Physical Therapy degree from an accredited program

Physical Therapy license in the state

Basic computer and EMR proficiency

CPR certification
