Job summary

FOX Rehabilitation in Portland, ME is seeking licensed Physical Therapists to deliver one-on-one care in homes and senior living communities. The Geriatric House Calls model offers autonomy, flexible scheduling, and meaningful patient outcomes.

Clinicians may use telehealth when appropriate, collaborate with an interdisciplinary team, and grow with supportive, clinician-driven culture. New grads welcome through Emerging Professionals Mentor Program and H1B sponsorship available.

Qualifications

• Licensed Physical Therapist eligible to practice in Maine.
• CPR certification required.
• Proficient in EMR documentation.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes, senior living communities, or clinics.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence, and function.
• Document effectively in FOX’s EMR system according to regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients and caregivers to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate to extend access.
