Job summary

Scylla Solutions in Los Angeles, CA is seeking a licensed Physical Therapist to provide in-home outpatient rehabilitation for older adults. The role involves evaluating and treating patients in their homes or in independent living facilities, delivering individualized therapy programs to restore function and promote independence.

You will supervise PT assistants as required, coordinate referrals, document care and participate in team care meetings.

Qualifications

• Must hold a valid Physical Therapy license in the state of practice.
• Master's degree in Physical Therapy or equivalent.
• Experience in home-based outpatient rehabilitation is preferred.
• Ability to evaluate, plan and implement therapy plans.

Responsibilities

• Provide in-home outpatient PT services for older adults.
• Assess environment, develop and deliver individualized therapy programs.
• Refer to other disciplines as needed.
• Educate patients and caregivers on therapy techniques.
• Supervise PT aides and students per licensure.
• Maintain documentation and billing records.
• Ensure safety and adapt plans for each patient.
• Collaborate with rehab team to select appropriate regimens.
