Job summary

Matrix Medical Network seeks a licensed NP/PA to perform comprehensive in-home health assessments. You will review medical history, medications and social factors, and coordinate with PCPs and care teams to close care gaps.

The role requires current licensure, board certification in relevant specialties, and the ability to travel extensively within the state. Office is on-site with field visits to homes and facilities; salary reflects market compensation and experience.

Qualifications

• Master’s Degree OR commensurate NP/PA licensure
• Current NP/PA licensure to practice
• Board certification in Adult, Gerontology, Family or Emergency specialties
• BLS, ACLS or CPR certification
• 1 year NP/PA experience
• Willing to travel up to 80-90% across state lines

Responsibilities

• Conduct adult/geriatric health assessments including history, diagnosis, education and referrals
• Administer point-of-care and lab tests as needed
• Collaborate with physicians, case managers, social workers and family
• Coordinate with PCP for education and follow-up
• Provide care at members' homes or SNFs
