Job summary

Jobtailor is seeking a Certified Advanced Practice Nurse Practitioner to provide in-home primary care in New Jersey. You will manage a panel of patients with chronic illnesses, delivering longitudinal care with high clinical autonomy and participate in on-call rotations to ensure continuous support.

Ideal candidates hold an APN credential, an NJ RN license, DEA/CDS registrations, and a master's or post-master's certificate in advanced clinical practice, with at least 1 year of NP experience.

Qualifications

• Located in or near Mercer, Middlesex or Somerset County, NJ; able to travel locally for home visits.
• Active and unencumbered NJ Registered Nurse license.
• Certified Advanced Practice Nurse Practitioner (APN).
• Master's degree or post-master's certificate in advanced clinical practice.
• Minimum of 1 year NP experience in a clinical setting.
• Active DEA and CDS registration.
• Current Basic Life Support (BLS) certification.
• Experience in home care or house call settings preferred, but not required.

Responsibilities

• Manage a dedicated patient panel, delivering ongoing in-home primary care.
• Care for members with chronic illnesses and advanced medical needs.
• Participate in a shared on-call rotation to ensure continuous support.
• Provide comprehensive primary and supportive care directly in members' homes.
• Provide longitudinal primary care for members with complex and chronic conditions.
• Focus on continuity, quality, and outcomes.
• Participate in ongoing care planning with a high degree of clinical autonomy.
• Build lasting patient relationships and manage medically complex cases throughout the continuum of care.
