Job summary

Matrix Medical Network is hiring for in-home comprehensive health assessments conducted in members' homes, including geriatric and adult evaluations. You will review medical history, medications, and risk factors, coordinating with PCPs, case managers, and caregivers to close care gaps during each visit.

With state licensure to prescribe, professional certifications, and strong communication skills, you’ll work in a flexible FT role offering travel to members’ residences and a sign-on bonus in

Qualifications

• Master’s Degree or commensurate NP experience and licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult/Adult Gerontology/Family/ Emergency NP specialties.
• BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license and auto insurance coverage.
• Experience with health record software.

Responsibilities

• Perform comprehensive adult/geriatric health assessments in members’ homes.
• Review medical history, medications, social factors and risks.
• Coordinate with physicians, case managers and care teams.
• Administer and order point-of-care and lab tests as needed.
• Document findings clearly following Matrix guidelines.
